Symptoms Screening Questionnaires (SSQ) are no longer required. However, if any of the stated conditions apply, you must return home and self isolate. Bowlers MUST NOW INITIAL BESIDE THEIR NAME on the ROSTER SHEET for the date they are in attendance. The dated Roster sheet will be attached to the inside of the Equipment Room door and is required for Contact Tracing.
Also, this serves as your attestation that you have answered NO to all the questions on the Symptoms Screening Questionnaire (attached to the door beside the Roster). Therefore, you each must initial the Attendance Calendar yourself and CANNOT do it for anyone else.
Initialing the Roster Schedule is not optional, it is the requirement for bowling at the Club currently. Should the Protocol change, you will be advised. Your continued cooperation is appreciated
