Accident or Incident Protocol

We have now included a Club Accident and Incident Protocol for the reporting of Health and Safety incidents occurring at our Club facilities, including the Greens, the Club house, and immediate surroundings. Should an accident or incident occur it will be a requirement to complete the attached JdFLBC Accident & Incident Report to submit to the Club Safety Officer. Completed forms will be reviewed and reported to the Executive for consideration of any follow-up action and will be retained for future reference. 

Blank forms will be made available in the office cabinet and should be completed immediately after the incident has been resolved. Completed forms can be left in the office and the Safety Officer is to be advised by email.
